Hair loss is one of the most emotionally difficult concerns patients bring to my office. PRP (platelet-rich plasma) hair treatment has gained significant attention over the past decade - and for good reason. It is a legitimate, evidence-backed option for certain types of hair loss. But it is not right for everyone, and I want to give you an honest picture of what the research shows.
PRP is derived from your own blood. After centrifugation, platelet-rich plasma - which is concentrated in growth factors like PDGF, VEGF, and IGF-1 - is injected directly into the scalp at the level of the hair follicles. These growth factors stimulate follicular stem cells, increase blood supply to the follicle, and prolong the anagen (growth) phase of the hair cycle.
PRP is most effective for androgenetic alopecia (male and female pattern hair loss) - the most common cause of hair thinning. It is also used for alopecia areata with encouraging early results. PRP is not appropriate for advanced scarring alopecias where the follicle has been permanently destroyed.
Multiple randomized controlled trials have demonstrated that PRP significantly increases hair density, hair diameter, and hair growth activity in patients with androgenetic alopecia compared to placebo. A systematic review in Dermatologic Surgery found mean improvements in hair count ranging from 20–40% after a treatment series - comparable to minoxidil in some studies.
The best candidates are in the earlier stages of hair loss (Norwood I–IV for men, Ludwig I–II for women), motivated to complete the full protocol, and not relying on PRP as a standalone cure. Patients with advanced hair loss or complete follicle loss are not good candidates - in those cases, hair transplant is a more appropriate discussion.
